Interprofessional Care

Better outcomes happen when we work together.

Interprofessional care means that different health professionals collaborate – with each other, with clients, and with families – to create a unified plan of support. Instead of working in silos, our team shares knowledge, coordinates goals, and tailors therapy so it all fits together.

OVERVIEW

Why it matters

For clients and families, interprofessional care means:

More support: you have a whole team on your side, not just one clinician.

Less stress: fewer repeated assessments, clearer communication, and one team working towards the same goals.

Stronger outcomes: different perspectives combine to create well-rounded strategies.

OUR APPROACH

How we provide interprofessional care at QPAH

How We Do It at QPAH
Our speech pathologists, occupational therapists, and music therapists work side by side, regularly meeting to share insights and align therapy plans. We also collaborate with schools, kindergartens, residential aged care facilities, and other community and health services to ensure continuity of care.

This interprofessional model allows us to:

  • Design therapy that addresses the whole person, not just one area of need.
  • Build collaborative programs (for example, speech and music therapy sessions together to support communication).
  • Ensure therapy goals are meaningful, consistent, and achievable in everyday life.
