To refer to QPAH’s services, please complete the below form and we will be in touch. We appreciate your referral.
A doctor’s referral is not required to access any of our services. However, if the client does not have private health cover and they are not under NDIS, they may be eligible for an Enhanced Primary Care (EPC) referral, to access up to 5 subsidised speech or occupational therapy sessions per year through Medicare. QPAH must receive EPC referrals prior to or at the first appointment. Music therapy is not covered under an EPC or private health insurance, but is fully funded under the NDIS. Regardless of the funding source, please fill in the below form.
